    Hi Cynthia, 

    We have a practice and education team the consists of 3 nurse educators, 3 clinical practice specialists / CNS, 1 outreach and telehealth, and 1 triage specialist. We work on projects across the division, but also act as consults to our clinic leaders in various areas and through out the system.  

    Our practice specialists primarily focus on risk mitigation, program development, policy development, compliance, scope of practice, role utilization (top of scope practice), EBP/QI/PI/research, team member development, and standardization of practice when appropriate.  Our CES role are experts at competency management, education and curriculum development, role development, orientation/onboarding, research/EBI/QI, and collaborative practice (with the organization and outside the organization. Our outreach and telehealth specialist works to standardize practice, operations and improve quality in these areas. The triage specialist guides review, development, and implementation of specialty triage guidelines while ensuring quality. Our roles have some crossover and we partner closely together as well as with our clinical leaders, direct care team members and SMEs.

    I am curious about how many practices your staff outlined above are covering, i.e. how big is your medical group?
    We are about 115 sites and growing, PC and multispecialty, struggling to find a good model to meet the needs but l like the concept you have outlined.

    Hi Luan, 

    You definitely have more locations than we have. I am not sure the exact amount of service lines we cover, but it is around 30 and growing, over 120 sub subspecialty, 8 network locations, and have about 600,000 clinic visits a year. For outreach, we have 22 different sites with multiple clinics at each across several states. Our model has changed over the past year as we have grown. We started with CPS and CES pairs. These two individuals would cover specific service lines, For example, I cover with a CES partner all our medical service lines and Primary care clinics. We have kept this model, but we have also had a CNS join our team recently that overs one location. In addition, one of our CES's has shifted her focus to Ambulatory wide program development. We have our own specialties where we are the SME of from a systems perspective also. We partner closely together to try to prevent working in cilos and work toward standardizing when possible.

    Forgot to answer your other few questions. We all all nursing and the majority of us are Master prepared. We all hold at least one certification and experience varies.
