Hi Jennifer,
I am the anticoag nurse for 8 primary care offices. I travel to 4 offices throughout the week to make it easier for the pt. I do not manage DOACs. I do create Lovenox bridges using protocols created by the providers. Currently I manage a little under 200 pts but have had as many as 250. There are still a few providers that manage their own pts warfarin, but it is less than 10%.

For those of you with RN managed anticoag programs - are you managing warfarin AND DOACs - are you doing bridging? If you are managing BOTH - what is your staffing ratio?
I also am curious - what is your % of patients nurse managed vs provider managed?
