    Hello, 

    Do any of you have orgs that bill for triage calls? 

    I want to start this at my org and wondered if I could connect with someone already doing this work.

    Triage by definition is non-billable. It was made even clearer in the Telehealth regs that came out. We got around this by hiring Telehealth Nurse Practitioners who complete an actual TH visit with the patient instead of it being reviewed by triage for many complaints...so, as long as it can be booked with them within a certain timeframe, we bypass triage. This has allowed us to convert some calls into direct revenue, and also improve triage call flow through the call center. Otherwise, triage is extremely expensive because it is non-revenue generating work. Yes, there is cost avoidance by hopefully reducing the likelihood of medical legal liability---but no direct billing for the nurse (unless you are a call center that offers triage as a service and you are billing the entity who contracted for your service). 

    You can track visits created by your triage nurses--but again, I think you will find, depending on call duration, complexity, etc. it is still expensive, and difficult to cover the full overhead.

    I agree with Mary.  This is non-billable.  If you are able to take a more value-based approach, nurse triage can be used to manage access - getting patients to the right level of care, protocolized care, or home symptom management.  That way your providers are not seeing patients that could be cared for in different ways.

    Tania,

    I work in a Nurse Triage Call Center independent of our healthcare system and practices.  Initially set up 27 years ago as an option for our pediatric population, we billed per call.  However, we stopped billing 20 years ago.  To show our value we report these calls as Community Service hours as we are a not-for-profit organization.

    So, while we are not revenue generating, the value we bring to our community is documented.  We have also demonstrated how our calls decrease unnecessary ED visits, which has another value in of itself.
