    At Virginia Mason Medical Center, we define Chronic Care Management as:
    "Our Chronic Care Management team provides Primary Care patients with individualized, telephone-based support to help them better self-manage their chronic conditions. The team consists of RN Care Managers, a Clinical Care Coordinator, and a Medical Social Worker."
