    I oversee a Rn run Anticoagulation clinic. We manage in-office patients along with home testers, patients that go to the lab, and patients that have Vna come in. My question is how do we bill for those phone-managed patients? Can we use a  93793?

    Good Morning,

    I also oversee a Nurse run AC clinic. We are fully tele-managed now. We bill 93793 for all INR's we touch.

    If you have a documentation template that allows you to document the management of and education for the warafarin management then you should be able to bill the 93793. When we are doing an At Home test review, we create what we call an AdHoc Clinic encounter to bill for the date of service. These AdHoc encounters for us allow the information to cross to our billing system.

    Hello,

    My organization's compliance department has not allowed us to use 93793 for anticoagulation services.  They have stated that to bill this code a provider needs to interpret the results and recommend treatment.  Has anyone else had this concern?  It is challenging when different groups interpret the regulations differently.

    We have a anticoagulation protocol that is signed by the provider and they sign off on the notes and must approve the any medication changes. We make sure it is signed off by a provider that is on site that day as well.

    Amanda would you be willing to call me as I have several more questions on this. Do you send the note to dr/provider to sign off on. Everything I am reading states for home tester it's generates an rvu and only billable by md/pa/aprn etcc..

    We have been notified that we are no longer able to use 93793 for our anticoag visits done by our RN's since it is limited to providers based upon our coding department. This was totally a surprise to us.

    I don't believe there was a document.  I noted that I had received guidance from our compliance department that this code requires a MD/APRN to review lab results and guide dosage.  In my organization RNs are managing most patients via protocol and we have been advised to use 99211.

    Agree-our group does that as well. This does require a standing order though in order to guide the practice by an algorithm. If your organization does require a provider to guide and determine next steps-the RN can still do the visit, and then just show the result to the provider and relay the info at their visit. They can then just document the verbal order from the provider and the provider can add a short addendum or timestamp a TE stating "agree." This way the RN can still bill a 99211 with the ICD-10 for the condition that requires them to have anti-coagulation.

    99211 for in person visit only right?   Or are you able to use those for telephone encounter?
