  • 1.  Chronic Care Management Concurrent Billing Codes

    Hello - We have recently started offering Medicare's Chronic Care Management to qualifying patients in our system. For those that have been completing this work how have you dealt with the concurrent billing codes for home health supervision and certain ESRD. Do you continuing billing for the CCM services or discharge patients that are being concurrently billed with these codes?

    I haven't had to deal with the ESRD codes yet, but in regard to hospice oversight. If they are being seen by the Hospice Medical director, we generally do short check ins but never have the time to bill, we want the family know we are here to assist if we can. We believe it is what is best for the patient.

    I'm just getting caught up on reading posts and I saw your question about billing for CCM services. If a patient is receiving home health skilled nursing we do not bill for CCM services. We kind of put them on "hold" while they are receiving these services. We do have several patients that are in our CCM program and palliative care and so far have not had any billing issues. Hope this helps, but I am so passionate about this subject that if you ever have any information on things that are working well for your program I would love to hear about it!
