Hello,

I'm a new clinical nurse educator and looking for tips/guidance on developing a mock code blue/emergency response drill for the ambulatory clinics.  We have several satellite clinics that pose a challenge for me to attend (over 90) so it was suggested that I develop a course that can be held every quarter and a super user would need to attend and then take this information back to their clinics.  It would be the responsibility of the clinic manager/supervisor to hold the mock code blue and I plan to have a refresher training in my annual skills day.

Curious if any of you would be willing to share what you do for your clinics?  I have an idea, but having a hard time starting this project out.  All ideas and feedback are appreciated!

We work with our enterprise resuscitation team and go out to the clinics every other month to do a mock code. We don't announce that we are coming. Obviously only going every other month we don't get around to all the clinics but is a start.

We organize yearly EMR mock drills for all of our ambulatory medical office buildings and share widely with our clinics the policies/procedures associated with EMR.

First, we meet with our departments to review their EMR response workflows. We discuss participation for simulated drills and pick specific dates to do our mocks. We develop EMR case scenarios for adult and pediatric patients to run on our scheduled dates then debrief with the responding team. Each department gets a scorecard after the drills as feedback for process improvement and/or to identify gaps in care.

We are a smaller organization but our process is: 

1. January is all RN's, MA's Lab tech's etc. get trained on code blue/emergency response at their all staff meeting  (we only have BLS carts not ACLS)
2. Each team is then required to do 2 mock drills during the year. The nurse is in charge of running these  Some teams do them unannounced some do not.  I provide the evaluation sheet and the scenarios.  The scenarios must be relevant to the population, e.g., Women's health clinic must do one precipitous birth drill.  
3. Since nurses are not  in our clinics at all times I also ask the MA's to go through the emergency cart annually to be a least a little familiar with it.  They submit their check off to their supervisor.  
4. This year I happen to have a per diem RN that went to all our sites and ran a drill with them.

We do surprise simulated codes in our ambulatory clinics on a yearly basis which is led by our ambulatory educators. We have 26 locations, we do not do them in every department in the massive clinics, which does impact the number of times we can do this work. When we have an actual event, we do a debrief with the team led by the managers and a quality review.
