Good afternoon,
My organization is interested in developing an extensivist clinic to care for complex patients in the critical time between discharge and PCP appointment. We have implemented a pilot where we see patients two mornings each week and would like to scale this. Would those that have already developed this resource being willing to share any protocols, business plans, staffing models and/or scope of services that helped with success. Appreciate any guidance that others might have.
