Good morning,
We completely revamped our Ambulatory Falls program a few years ago. We use yellow magnets outside the door and yellow wrist bands for outwardly visual cues and we also have a Fall icon in Epic that automatically pops up in the DAR and Provider schedule when a screening is scored as high-risk. That Fall icon remains there for the entire day through multiple visits if needed. In addition, Falls education on staying safe at hoe and staying safe in clinic automatically gets put in the AVS for all patients that are screened as high-risk, the MA also quickly reviews the staying safe in clinic during rooming and it's posted in the room.
