Does anyone have Fall Policy or protocol in Ambulatory clinics?

Our organization has a dedicated Ambulatory Falls Workgroup, where RCAs and action items are reviewed, discussed and operationalized across the system. There is also opportunity with the workgroup to develop system-wide policy.
