    Hello,

    For those who are doing population health, what is your process for mailing stool based tests in bulk to patients-is it done by your medical assistants? It would be great to know what others are doing.

    Hello,

    We have our analytics team pull patients who have outstanding colorectal cancer screenings to create a roster. Then our unlicensed, non-clinical staff call those patients initially to get them scheduled for colonoscopy but if patient defers then a fobt kit or cologuard kit orderd and sent.

    Thank you Roxanne! So do you offer all patients colonoscopy (even average risk)?  We are offering average patients stool based testing first. Also, for the kits you mail, are they pre-paid?

    Your welcome! We do offer all patients colonoscopy. We have such a large patient base that we don't have the bandwidth to filter patients based on risk at this time and yes, the kits are all prepaid.

    We recently wrote a protocol to allow our MAs to work the list and send out kits.  See below: 

    1. Select patient.
    2. Open patient chart in Epic (must be in a clinical department login – not admin, dental or SBHC).
    3. Review Health Maintenance for CRC screening need.
    4. Review for history of colonoscopy. If patient has had a colonoscopy, they are excluded. Add to Exclusion list.
    5. If there is an active CRC screening already ordered and the order was placed LESS than 90 days ago, re-print this active order and close chart.
    6. If there is an active CRC screening already ordered and the order was placed MORE than 90 days ago, cancel this order. Proceed to order a new CRC screening.
    7. Add an order
    a. 82274, Fecal Globin by Immunochemistry
    i. Diagnosis code: Z12.11, Screening for colon cancer
    b. Sign the order under supervising provider (cannot be a naturopath provider for Medicare patients)
    8. Document the following in the patient note:
    a. "Patient is due for a CRC screening. Reviewed patient chart to confirm patient is due. Placed order for CRC screening per Clinical Protocol." For CRC mailing: add "CRC screening will be mailed to patient's home address on file."
    9. Navigate to the "Open Orders" tab.
    a. Find the FIT test order you just signed.
    b. Select "Release". This will automatically print the order.
    10. Close encounter by signing the visit.

    The MAs are sending them out and lab company includes a prepaid envelope.

    HI

    We have a similar process to Angela's team but not always using MA's.  Several years ago we participated in a study and found that sending the kits alone had low response rate.  Sending the kit with a follow up call increased return rates.  Pts are given the option of sending test to lab via mail or dropping off at clinic.
