  • 1.  Medication Review/Reconciliation

    Hello friends,

    My organization's quality committee has recognized that we have opportunities to do a better job ensuring medication review and reconciliation is completed in a meaningful way.  I serve on the committee tasked to make a proposal for a better process and would appreciate hearing your organization's success stories.

    1. What is your organization's process to ensure that medication review and reconciliation are done? 
    2. Do you have different processes for primary care and specialty care? 
    3. Do your providers reconcile medications they have not prescribed?  For example, at my organization we have primary care providers who are not comfortable removing a medication that was prescribed by a specialist on the patient's word and specialist that are reluctant to remove medications that do not pertain their expertise. 
    4. What were the pain points recognized by clinical staff and providers?
    5. If you have instituted a change, what were your lessons learned?

    Good morning,

    I would be interested in any successes in this complex issue also.

    Seems like no one takes responsibility thinking someone else will address and yet it is such an important task because medication issues contribute to admissions,  readmissions and poor outcomes.

    Health literacy also plays a part in how the medication review needs to be done..

    I am interested to follow this topic, as well. We have the same struggle with providers who prefer not to confirm or deny medications that they did not order or are not related to their specialty.

    we have had a lot of pushback from providers on medications they did not order.  Especially if its a specialist office not wanting to reconcile the entire med list.  We have made it part of the standard rooming process and its the expectation that each med is reconciled at each visit regardless of who ordered it.  What we have told them is they are only verifying what the patient is taking.  If something is ordered by another provider and they are not taking it because they don't like it or can't afford it, etc. We tell them to follow up with their ordering provider and leave it on the medication list.
