I used this template for a while, but it didn't really catch on in our department and I eventually stopped using it in favor of more free-form notes.
The thing I like about it is that it identifies the triage nurse who wrote it, what clinic they work in, and their direct line in case someone from another department wants to reach out with questions or instructions.
I used the "cc" to denote to whom I reported the triage information.
