    Thank you for touching on a vital action. Do you have a model you are working from? Just curious about how this will be a nurse driven initiative? I would like to hear from others on how to operationalize this clinic.

    I'd be happy to share our protocols and discuss with the group on a call. I shared with Janet individually, but there is a lot of research on this topic and it is a passion project of mine for our organization. Currently, we have protocols for MFM nurse visits and anticoag clinic visits for cardiology. The process is kind of long but well worth it to increase access!

    I just sent you a private message from Annette and myself. We would love to set up a time for you to share this on our NSI SIG Monthly Networking call. We like to check in with our presenters first to be sure NSI's are clearly delineated and standard (Donabedian model) applicable. If you presented at conference, I missed it (so many incredible sessions to choose from)! I also haven't had time to see any of the recordings. This is exciting work!!

     

    If anyone else out there has what they believe to be NSI exemplars and are interested in sharing on a national level, please reach out to us in this forum.

    We have established a robust Nurse-Led RPM Hypertension Management Program and have enrolled over 1800 patients so far, focusing on our more vulnerable populations in the south side communities. Our nurses can nitrate up to 3 anti-hypertensive medications, provide support and education, and can get patients seen sooner when there is an issue. We started in our main primary care clinic on our medical center campus and have expanded to nephrology and cardiology. In the cardiology clinic, the nurses use an adapted protocol for in-person visits.

    This program was a story in our most recent Magnet document and was presented during site visit, really well received by our surveyors.

    I’d be happy to share with anyone interested.

    We also have a program for RN Titration.  It's referral-based from providers.  I have a question as to how you charge capture for these visits.  Do you charge or do you utilize a no charge code?

    Hi, Laurie

     

    I am aware of Care Managers embedding HTN management into their care practices and time-tracking using CMS billing codes for Care Management. This is an excellent avenue as it provides more evidence/outcomes r/t RN Care Management roles.

     

    Also, some people are charging Nurse visit codes.

     

    We must contribute to revenue in all avenues possible in order to speak to our practice as RNs. We are not a "bundled service." Our cost as a profession is not the same as equipment and office supplies. We need to start including revenue contributions along with patient outcomes r/t our practice if we are going to survive as a profession.

     

    I am so proud of so many RNs who are leading this work!

    I work with Care Management teams throughout the country. We developed a Nurse-led Evidence-based HTN protocol using the American Heart Associations (AHA)  Hypertension Toolkit for professionals. Nurse Care Managers use this with patients engaged in Care Management. They use time tracking reimbursement via CMS billing codes. Patients receive evidence-based, holistic HTN care, have better outcomes "and" RN Care Managers are able to contribute to practice revenue which promotes RN ability to speak to outcomes directly related to our practice. As well, providers in clinic have more time to manage the more acute needs of patients.

     

    Check out the toolkit on the AHA website. It was designed for our use.
