    Good day,

    I want to advance nursing in ambulatory care and am wondering if anyone has had success with nurse led annual wellness visits. We are working with HCC scores and trying to increase revenue in primary care, and looking for guidance.

    I'm also interested in this topic. I'm currently focusing on how we can report the added value that nursing brings to primary care on a monthly basis. It seems that nursing is often viewed more as a cost. Does anyone have metrics they report monthly that could help highlight this value? These two topics intertwine.

    We have developed a RN led annual wellness program that has been quite successful. Currently we have one nurse who does annual wellness visits as her primary role and several others who do them as part of their role. The RN does the scheduling, communicates with the patient directly about the visit, does the visit and any needed follow up. The note is forwarded to the primary provider for signature. The signing provider must be in the office the day of the visit. If the primary is not available, the covering provider signs.

    We instituted the same process in our family practice in upstate New York. An MD must be in the office at the time of the RN Annual Wellness Visit. There are financial reimbursements from Medicare and per our data specialist, the visit Attributes the patient to our practice. The challenge has been in scheduling the visits to balance with the RN availability as well as the willingness of the patients to participate. We tried scheduling people to coordinate with their Lab appointment but recently the organization stopped scheduling labs so that changed things quite a bit.
    On a personal note, I just went to my annual visit with my NP provider, and being new to Medicare, she immediately turned it in to Medicare Wellness Visit and sent her nurse back  in to do the Nurse questions for the Wellness portion. This is true provider buy-in but in our own organization it has been met with resistance...God only knows why.

    Any time the RN's can perform independently and make an impact on patient education and care it is a win.

    We plan to begin nurse led AWV next month. Do you incorporate HCC documentation or CDI prompting in this process?

    Hello,

    Our Organization has recently stood up a nurse-led Medicare AWV program. The program has been instrumental in improving health outcome, SDOH and provider access in addition to addressing care Gaps. We have had some success with a few nurse-led programs such as nurse visits for chronic disease management, Medicare AWV and Anticoagulation.
