    Hello,

    I am a Nurse Manager for a non-for-profit organization with 3 FQHCs in Chicago, IL. 

    Our clinics have a mix of MDs, NPs, and PAs. I am looking to implement standards of RN to provider Ratios, in support of hiring more FTEs. 

    I appreciate any guidance of where I can find the standards of care and RN ratio guidelines for the ambulatory care setting or any recommendations from your organization.

    First off--hello! I am the CNO of a large FQHC in CT--always happy to see another FQHC partner on the community threads!

    1) for nursing ratios---ours are 1 RN to every 2 provider panels for primary care support--then we have centralized nursing departments for triage and a few in our pop health department. This are in addition to the routine ratio. We are able to bill 99211 visits for our nurses, so this assists to cover the overhead of the nurses along with many value based contracts. 

    2) I am putting together a request to add an FQHC sig for members...interested? I can add your name and contact to my list--once I get to 25, we will submit the final proposal to the board to have our SIG added!

    3) shameless plug! I am the only candidate for board of directors from the FQHC world--would love to have your vote! we are a large voice and represent roughly 1400 clinics nation wide--so, really want to make sure the board has representation :)

    I have heard the east coast is able to include nursing for collaborative billing, however, I am in Chicago area and we currently do not have the billing structure to support this. 

    Yes, I would support a FQHC sig.

    Good morning, 

    I work for an FQHC in Vermont and would love to hear how you are billing nurse visit. We are finding a huge need for these, for either education, wound care support, or any other need .

    Our staffing currently for most of our sites is that each provider has an MA assigned to them, there is a RN (1-3 depending on clinic size ) and then a RN care coordinator for each clinic (this puts 3-5 providers to each care coordinator)

    What a great idea to have multiple FQHCs is a group together

    I believe that Mary Blankson ,who is on the AAACN Board, had originally offered to get the SIG started once we had 25 people responding. We have far surpassed that number.     I would love to explore Nursing staffing ratios to provider panels that would include LPN (Panel Managers) as well as RN's who bill for services as well in the FQHC setting/primary care. Our 8 clinics range from 4,000 up to 9,000 clients serving the underserved, uninsured, underinsured and insured clients including the refugee clinic, HIV, family practice, internal medicine, pediatric and specialty practices. Our nurses serve a wide breath of care delivery including the call center. Therefore I am truly intrigued:)

    Melissa,

    Within the VHA, the RN Care Manager staffing model includes having 1 PCP (MD/DO, FNP or PAC) paired with 1.0 RN, 1.0 Clinical Associate (LPN, LVN, MA, HT or ICT) and 1.0 Clerical Associate for approximately 1200 patients. This staffing model was built to allow for continuity of care with team, same day access in fully staffed high functioning teams, and each role functioning at top of scope with clear role clarity. Suggest starting by developing a staffing methodology guidance document to show what you currently have and how that coverage has gaps paired with suggested solutions for the gaps to enhance efficiency, patient experience and patient outcomes.

    Thank you for the question Traci,

    The Primary Care RN's triage patients that present to the clinic in person for their assigned teams, the Clinical Contact Center triages patient telephone calls that come in via telephone. There are Care Coordination Agreements (CCA's) that are established between the Call Center staff and the Primary Care/Ambulatory Care teams to help streamline the flow for patients. In addition, the CCA focuses on role clarity between Call Center and the Clinics.

    Good morning, happy to chat re: nurse provider ratio. Prior to Covid we had a great model in place- struggling to return there post-Covid. We are a five center FQHC system in PA and we had Clinical RN coordinators ( one per site except for the smaller sites then it was one per two sites). Additionally, we have RN Care Mgrs- one per team ( three full time providers/ team). We utilize LPNs as care navigators as well as to support med refills and direct clinical care. 

    Right now though, I am the senior nurse and I supervise all clinical support with assistance of "lead MAs". I have one RN and one LPN in clinical with some PRN staff supporting med refills and patient call backs/ triage.  We are going to be identifying the LPN as a clinical nursing advisor to provide more direct CMA supervision. 

    One RN in care mgmnt right now for our highest risk patients and an outsourced contractor ( Chartspan) to support the CCM efforts. 

    This is not the best model- the one pre-Covid was way stronger. The ratios we had been using had been designed based on a team-based care model.

    We are moving towards team based care model as well.  We have a 12 site FQHC.  We have one RN per site or 1 RN over two site if the panel size of the provider is smaller.   We are following the MGMA model staffing recommendations.  We do not have any LPN or if we do they are acting as MAs.   To help with our VBC contracts and give RN's more "value" we are enhancing their role with RPM, nurse visits for HTN and Diabetes Education (billable visit if ACDES certified).  We are also increasing RN's in our organization by allowing them to do MAWV to increase encounters for providers and do wellness visits.  RN in the FQHC is so valuable especially in the arena of VBC.  Unfortunately in the FQHC it's so hard to find good quality RN's.  The other thing is it is hard to show value in the RN in way of how they impact your VBC contracts but also generate revenue as it is left up to the providers to do so versus non FQHCs.  So we are balancing their role in promoting quality changes to meet our VBC goals and provide nursing visits that are benefiting the VBC goals and revenue generating to justify their extra expense.

    About RN ratios, check your state board of nursing. In NY there is no standard RN to provider ratio, however one RN can only supervise 3 LPNs when doing immunization clinics under the non-patient specific standing orders laws.

    Hello, I am Director of Nursing at an FQHC in South Carolina, and I would like to be added to the FQHC sig once it has come into fruition. 

    We are currently staffed with 1 RN charge nurse, 1 Staff Nurse (RN or LPN) and 1 Medical assistant to each provider team (MD, NP, or PA). Am also looking and considering ways to maximize each role to their potential. Interested to learn or connect with those in FQHCs who are using RNs for DSMES and MAWV to generate billable encounters.
