We actually have 2 different programs for pre-visit planning. The main one is for the majority of our primary care and all of our specialty offices. The other is for a smaller group of offices that are part of our Care Model Redesign (CMR) program. The main program uses a SmartForm in Epic. We have customized SmartForms based on the needs of specialty sites. The smaller group is using SmartPhrases to document. I have attached our guiding documents. We teach a virtual course for both programs during the 3rd week of onboarding, though new hires only attend the session that is applicable for their office. The course is 2 hours and includes an independent patient assignment where the new hire will have the opportunity to pre-visit plan for a specific patient.
The goal for our medical group is 85% completion. For the main program, we have a report we can run based on the office and specific provider. The smaller group's reporting is proving to be more challenging, and currently we are not able to compile a completion rate.
