Hello, 

I am looking for a job description to create a team that will help support our Clinically Integrated Network with Value Based Contracts. This team should review attributed patient charts to find supplemental data that closes open care gaps, upload the supporting data to payor specific sites, complete patient outreach calls to encourage patients to complete care gap appointments for preventative screenings (that have already been ordered by the provider), complete patient outreach calls for non-filled medication adherence metrics, communicate to prescribing providers regarding best practice orders for medication adherence, and communicate to providers regarding other quality metrics best practices and workflows to help close open care gaps. 

This role will differ from the clinical RN Care Coordinator, Health Coach, or Pharmacist on the Population Health team, and be more of a non-clinical role.

I would also welcome any additional feedback on how other health systems support value based contract work and closing care gaps with a non-clinical team.

Hi Jessica, Here is an example of our Care Coordinator role that does much of our work in Value Based Care and coordination of closing care gaps ect.. This role is not a clinical caregiver. We really have focused on them having good customer service skills and the ability to problem solve See attached example
