    Hello - 

    We have been completing Medicare Annual Wellness Visits for over a year.

    We are looking to continue to enhance our current process and continue to allow our nurses to work up to their highest scope. We currently have a small list of orders that our nurses are placing and signing. Does any one have a standing order set for the RN led visit?

    Currently our RN visits are occurring a few weeks prior to a clinician visit, has anyone decoupled the RN visit from an associated clinician visit?

    Hi Emily.  Yes, we are doing the same with setting up a process to have the sequential AWVs done by RNs.  I am in the middle of writing a policy for protocol orders allowing RNs to be able to place orders as a result of identified care gaps.  That order will go to the primary provider for signature.  We also have a dedicated NP in our primary care office who will sign them off as well.

    Does the patient have a visit with a clinician in connection with the RN visit? i.e. within a certain time always from the clinician visit? 

    Also how are they getting the orders to the clinicians for signature?

    The patient can see a clinician on the same day if it is warranted (the provider has to add a modifier 25), or they can see them for a subsequent appointment to further discuss their care and chronic conditions.

    Hi Stacy, I am wondering if you have completed your nurse initiated orders for RNs to be able to place orders as a result of identified care gaps. If so, would you be willing to share that document? Does your EHR have the order sets built in or how are the orders placed by the RN?

    We just started RN led visits.  We do a pre-visit huddle with RN and PCP and only schedule follow up PCP visits as needed.  We don't have a standing protocol but I like that idea. Our population health team screens in advance for care gaps. For billing the RN completes the visit but does not sign it. The provider reviews the visit notes, signs any orders/meds refills and documents they approve the preventative plan of care and signs visit.

    One thing that maybe everyone else figured out super easy but took me awhile, is if the provider actually has to lay eyes on the pt.  I could not find anywhere in the CMS rules that this is true.  It seems only to be true for Rural Community health clinics.  We are an FQHC and the provider must be in the building but not in the room.

    Some tools we developed were: workflow (for RN or MA assisting a provider) pre-visit worksheet, and EPIC dotphrase.  We also matched the pre-visit HRA form we send to pt's to line up with the questions in the EPIC flowsheet.  It's an EPIC letter template so it pulls in all their meds and hx and asks them if what we have on file is correct.

    Greetings 
    CMS regulations do not specifically state that this is an RN billable visit.  They allow for clinical staff that have the competency to provide the services to perform them.  RNs have the education to provide this level of assessment, planning, and intervention.  The service is still billed incident to a qualified health provider (MD, APP).

    When we perform RN-led visits, the provider has to be in the same clinic, and the note is sent to the provider to cosign.   The provider does not have to see the patient, but they do have to sign off on the note, and it is billed under that provider.

    Billing

    Part B covers an AWV if performed by a:

    • Physician (doctor of medicine or osteopathy)
    • Qualified non-physician practitioner (NPP) (physician assistant [PA], nurse practitioner [NP], or certified clinical nurse specialist [CCNS])
    • Medical professional (including health educator, registered dietitian, nutrition professional, or other licensed practitioner), or a team of medical professionals that a physician directly supervises

    I find it's easier to think about it in terms of "providing" and "performing" . The RN can "perform" the First and subsequent AWV (not the Welcome visit) but only a provider can "provide" (bill) the visit.  If you want to use a G code the provider must be present in the building and must sign off on the prevention plan and visit but they do not actually have to lay eyes on the pt.  That is how we interpret "oversight". We have a brief pre-visit huddle with the provider to review care plan, prevention plan and address any needed lab, medication or care gaps.  We do not use the 99211 code for this visit.  If the provider is not available the day of the visit for some reason, then another provider must assume responsibility.  Many of our pts are fairly complex and require a follow up provider visit to manage a chronic condition.  

    We have not been doing this very long so take that with a grain of salt. I'd be interested how RN's sign the chart with the G code instead of the provider.  Am I making it too hard?

    I have also separately emailed leadership at AAACN asking if there is a consultant available to the organization who can advise for the variety of coding and billing questions that come through the list serv. No response just yet.

    Hi Leslie -  I am happy to see the continued conversation and appreciate you reaching out regarding billing/coding.  The RN led Medicare Annual Wellness visit is already a visit we provide within our system. We are trying to improve our process. Currently our RN's only sign very few consult orders for the patient based on information gathered from the patient during the RN visit (Consult to Advance Care Planning, Nutrition, etc.). I was wondering if any RN staff were signing any preventative care orders based on a standing order?

    Also we currently "couple" our visit with the provider visit. The RN sees the patient for a RN MAW visit then the patient has a visit with the provider 2-4 weeks following the nurse visit to address any chronic disease management and these preventative care orders that the nurse can not currently sign. We would like to explore allowing the patient to be seen when needed instead of having these visits together.

    Hi Emily,

    I do annual wellness visits, but mostly as joint visits with providers. They like that I can save them time by completing the AWV assessment. The thing is that some of our providers really value the the AWV as a time to catch up with their patients and don't want to give up that visit to an RN. This is a work in progress.

     I have done some independent AWVs before. My least favorite are for younger Medicare recipients who can't answer the assessment questions. Some of them come with a guardian and expect a physical and it takes some educating around the topic. These can be a struggle. 

    I enjoy being part of the care team, but this is a work in progress to get the RNs to work more independently.

    We are doing nurse AWVs. While having an independent nurse schedule is ideal, we have gotten only as far as the "coupled" model with the provider visit. Nurse only does subsequents, no initials. We are on Cerner and have built a Quickvisit for which the nurse can select orders including the G0439 and goes through without having to "propose" for provider to approve. This privilege is coupled with standing orders and AWV protocol for the nurse to follow.
