  • 1.  Screening Questions

    Hello,

    I am a clinic educator who supports several specialty clinics.  We are currently reviewing our screening questions that are asked during every patient appointment.  We are wondering if we are obligated to ask these screening question during every visit, or could we ask them at different intervals, for example, maybe every other visit or every 6 months, etc.. Patients who have frequent appointments voice their irritation at answering the questions at every visit. 

    The screenings include:  travel and exposure, learning readiness, abuse/neglect, fall, nutrition, and sometimes PHQ. 

    How often do your clinics ask these screening question?

    Hi Barbara,
    This is often determined by organizational policy. We ask the travel and exposure question at every visit, but most of the others are annual screenings in our organization and don't need to be done in specialty if we can see it was documented within the last year by primary care. Hope that helps!

    Hi Barbara,

    We ask travel and exposure when scheduling and at check-in at every appointment, even virtual/phone.
    Learning readiness, abuse/neglect, fall, and nutrition is usually only asked at annual wellness. Or yearly if no annual wellness done.
    We do a PHQ9 and GAD7 at every appointment.
