I am looking to connect with other nurses who have implemented a team-based care model in a busy primary care/specialty clinic. Has anyone done a QI project on this using some of the ideas from the AMA best steps forward and implementing pre-visit, intra- visit, and post-visit work? If so have you been successful and would you be willing to share some of your experiences in working on this project.
