Telephone Triage in EPIC

    Good morning,

         Our hospital is transitioning to EPIC and IT asked me if we know of anyone who does telephone triage in Epic. We have a charting system that our IT built specifically for us. We are an after-hours service for over 100 Pediatric practices that are obviously not all on Epic. So I am reaching out to see if anyone uses it currently. I saw online that Epic appears to have a program that can be used with Barton Schmitt protocols and have reached out to them but I would also like to hear from someone who actually uses it.

    When an Urgent Care Clinic rolled out EPIC, no triage section was purchased in 2010. Since I taught RN triage, I went thru EPIC with their trainer, NOT the hospital's IT guru and found the triage module was very appropriate for our patient population which included infants and children.

    Each module were an additional cost, which may have been the reason for not purchasing it. The variability in triage outcomes and costs of negligence lawsuits would outweigh whatever the module cost. 

    IT departments are usually quite adept at modifying a module to fit the clinical specialty.

    Thank you. We are strictly pediatrics and currently use the books, so I am hoping the cost is somewhat comparable. If we do not need the books we can just use the online version. I just wanted to hear from some people who have actually seen or used it to know how user-friendly it is. With change comes resistance and I do not want to change to something harder than what we have, if possible.

    Thank you. I think our IT wants to stop having to update the application they built for us and feel it would be easier if everyone was in Epic. So he asked if I know anyone who is using it and I said no but I know the group to ask.

    Understand completely.

    Used the EPIC module, triaging a number of patients and it was fairly easy to use. Like everything, whatever you use will require training and support. The latter being a BIG need and for longer than the leadership ever envisions. Good luck in your journey.

    Hi, we recently went live with EPIC and I don't recall there being a module for triage. Can you share a bit more info? The only only thing that I was told is that we can embed ClearTriage into EPIC but from this thread it sounds like EPIC also has their own module.

    You'd have to talk with EPIC as they were the ones to show me the triage module in the EPIC software. Our IT people didn't have a clue about any other modules clinical people might need or want. That's the problem in my opinion in establishing anything within EPIC.....it's the EPIC personnel that know the system and can speak directly to the clinicians' need for accurate documentation. 

    The triage module is an additional cost which the leadership fought tooth and nail NOT to purchase. You may need to make the case for cost savings by purchasing the module vs a work around or another off the shelf triage module. 

    It's been about 10 years since I evaluated Epic's triage module and I was suitably impressed by the thoroughness of the content at that time. I've taught RN triage extensively over the last  30+ years and reviewed quite a few negligence cases of failed triage in the past. Looked at the triage module from these perspectives and felt it was well worth the funding. 

    The leadership of the Urgent Care Clinic didn't feel the same way, unfortunately.  Perhaps after writing a number a checks with many zeros in cases where the patient was harmed, they might change their stance.

    My organization will be transitioning to EPIC in 2023.  I would be very interested in discussing your journey and training of your staff.   Triage is an area that my organization even without this transition, lacks adequate training in, and I would love to pick other people's brains regarding Triage training in general and EPIC and what it has to offer.  We currently use an OB Triage book, a Peds Triage book, and a general triage book.  Many of my co-workers don't use them at all or efficiently, because we are understaffed many times, I am not saying we do not triage well, but I would love to design a better triage system and training especially for our new grads that we hire to help them build confidence in their skills.

    There is a triage call function within EPIC, you do not have to purchase a separate program to embed into EPIC like ClearTriage, but I have heard good things about that program also.
    It runs with the Schmitt & Thompson decision support tool (the same as the book protocols).
    It has adult and pediatric, office hours and after-hours protocols.

    We have been using Epic for triaging for years and everything works fine for the most part. We have an analyst that updates the protocols in Epic when they are sent to us.

    Was there a call set up to discuss?  I don't recall.  We have been using this product since late 2016/early 2017.  We also contract with an afterhours service that uses the AH product.

    We use the Schmitt-Thompson protocols for pediatrics and adults embedded in EPIC. It works fairly well, and as others have stated it does require training and support. Currently we use LVM and Barton Schmitt protocols for charting.   All of the callers are not hospital patients.  Does EPIC allow for caller that are hospital patients?

    We use the epic telephone triage protocols across our organization as well. They are the Schmitt/Thompson protocols, updated by our IT when needed. Every triage nurse gets the hard copy books as well. Our Epic IT team have made some suggest modifications for charting so that the notes don't become too bloated for the providers.

    We have a section in Epic that all protocols can be accessed for review – might save money and be an alternative to purchasing the books for everyone.  We rely on this during training and chart reviews.  It is a great option!  We also use the parent education sheets provided with the package to send to families via MyChart when appropriate to follow up on a phone conversation.

    Hi Peggy,
    Our Epic team modified our notes by not having any of the questions that were a negative or a no in the background in Epic. So that we can always have them go back in and retrieve if we even needed to. That way the note is much shorter to the provider. Most of our staff clear the :Answer assessment" that you can F2 through to answer the questions, and use the questions to guide their shorter note covering  all applicable/positive questions. 

    All the no's used to go over into the providers note. Now only a yes will. 
    Additionally the care advice shows up as a drop down for the provider to view if they want, but not showing up in the body of the note.

    Good morning Jeannine,
    Our organization went live with Epic last December.  The Schmitt-Thompson protocols were inbedded in the triage application . When starting a triage call, there is a Triage Call tab that is chosen. There are protocols for office hours  as well as after hours phone calls for adults and pediatrics .Its very easy to use. We also had an option turned on that notifies our ED that the patient is heading there if that is what the protocol suggests. There are two training modules for triage that we use. They are the Triaging a Simple Call and Triaging a Complex Call. These were provided by the Epic Team.

    That will be great for our ED. Our IT built a system for both our bed placement and us... it is very similar but also very different as the tasks are so different. The bed placement board will definitely be going away since Epic has something that will do all of that. So they want to get rid of our "board" too so they don't have to keep maintaining it. So being able to alert the ED from within the system will be great.

    Our organization in Maine just transitioned to the Epic Triage system and it has been challenging for us. They put the triage protocols from Barton and Schmidt into Epic. Those are the same books we used. There are pros and cons. The calls take longer and sometimes there is no protocol for the type of call we get and that is pretty time consuming. I would say that a benefit to the system is that all of the home care advice is at our fingertips (when we can match it with a protocol) and it allows us to send advice to the patient through their My Chart account. 


    The system is a work in progress. The biggest problem is that we didn't have enough training on it and it has now been six weeks in the new system and we have really struggled. Over time, I am sure we'll get used to it.

    I would say the only problem with Epic triage is the Care Advice that is displayed is not only for the specific disposition chosen.  Epic does nothing to clean this up after the guidelines are sent to them from Schmitt/Thompson Clinical Content.  This makes it very difficult for unseasoned triagers to learn triage and use the guidelines as intended.

    The other thing I would say is that they are not effective for specialty.   They are great for general practice.  I would appreciate it if EPIC would partner on getting some specialty modules in where they exist but I have not heard anything on that front.

    My Informatics Nurse has had a request in with Epic for over two years to get this changed to only include the care advice for each disposition and not ALL the care advice.

    One can only keep hoping.

    We're an after hours program that uses EPIC and the triage module.  It does have some Cancer protocols, pediatrics and then the general DST (over 1000 of them) build right in so you do not have to purchase additional Clear Triage programs.

    I would love to network with other institutions utilizing the EPIC nurse triage module, or if it makes more sense to expand with Clear Triage?

    We just updated our "navigator" and I would like to hear how other organizations utilize the triage portion.
    Maybe we could coordinate a ZOOM meeting share ideas and see what is offered and available.

    My organization uses Clear Triage. We are not on Epic-and so I am not familiar with how often they update, etc. I do highly recommend ClearTriage-and if you are a federally Qualified Health Center, or have a segment of your organization that is, you get a large discount on the product. The product itself is also very inexpensive-and they also offer a great program for non-clinical personnel where you can create a tool to help them sift calls appropriately to triage or to another staff member based on your designed protocols.

     

    So, feel free to contact me if you have questions about that-I would love to hear what others say about Epic's software, because there may be a chance we change to them at some point as an E.H.R., and so, it would help me to know whether this is a product I would like to use. (and also which product is cheaper! Given that it is really no problem to use the web-based ClearTriage outside of the record-I am sure that Epic charges something for that module!).

    The Schmitt Thompson telephone triage guidelines are reviewed and updated annually by an interdisciplinary group.  The updated guidelines are sent to a contact within your organization that is responsible for the maintenance of the EPIC telephone triage module and it is their responsibility to upload the updated telephone triage guidelines.  If a new guideline is developed urgently such as the Covid guidelines that were developed and constantly being revised, then a separate notification is sent to inform customers/users that a new guideline has been developed and released and is available for implementation within your organization.

    We use Epic triage for our Daytime triage and specialty clinic triage calls.  Our Primary Care triage RN's use the Schmitt content and  specialty clinics use our custom specialty guideline content.

    Good Morning,
    A very active topic here that I had no idea was going on in the Open Forum!  I requested a Zoom call be setup in our Telehealth SIG as there is some great discussion here.  Details to follow on when the call will occur.

    Reading over the many post some highlights I noticed:
    • Schmitt Thompson protocols - imbedded in Epic, third party vendor such as Clear Triage, or paper/book  
    • Displaying all protocols responses or just the "Yes" response.  Are pertinent negatives clinically important documentation versus readability for provider?
    • Care Advice - Generic versus specific to individual protocol.   Are they verbalized to caller or also sent via myChart/email?
    • Focused Assessment (I might of missed it in this thread) - possibly the most important part of a Triage, how/where is it documented?

    Our Health System went live with the Nurse Triage part of Epic in May. Our pediatric teams LOVE it. Our adult teams are coming around. It was quite a change from just a blank telephone encounter. As we continue to develop this, I would love to chat with others who have/are using it to maybe create better education and tip sheets for success.

    Our hospital has EPIC and our health system integrated the Schmitt-Thompson nurse triage protocols into our EPIC system.  I know there are some pediatric protocols but I work in OB/GYN so do not utilize them but our family practice offices use both adult and pediatric protocols in the system. I would be happy to try and answer questions.
