    Do you currently have a role in your wound care clinic that coordinates program policy and procedure etc for the service line between the clinic, and care provided in hospital, by the same provider/provider group?

    Our in-patient and out-patient wound teams are independent of each other and run on different service lines. Each is represented on our shared governance Wound/Pressure injury prevention committee, which has an inpatient focus.
    Our inpatient wound staff nurses identify wound providers
    on their initial visits and if possible and appropriate continue the treatment plan. There are limitations as inpatient wound nurses only are involved with patients who they are consulted on.
    EPIC EMR has made this easier. We have though identified issues with AVS not containing discharge wound care instructions.
    Providers need to input the treatment plan and we find this is frequently not the case.

    Continuity of care from inpatient to outpatient is not fluid in our organization.  Wound clinic is offered once or twice a week at our 3 largest clinics that offer specialty services.  The providers at these sites rotate through this specialty once a week and function as primary practitioners the rest of the time. We advocated for a Wound Educator given the needs of our patient population and lack of wound education support. They joined our team earlier this year and support inpatient and the outpatient wound clinics. We are working with them to create wound care workflows; particularly around Compression Therapy and Unna Boot Therapy.  There is definitely a missing link.
