  • 1.  Advance Care Plan - what do you measure?

    Hello, 

    We are currently in the process of refining our definition and success metrics for what constitutes as an advance care plan within our organization and are curious what criteria other organizations are using when pulling this metric/data?

    Specifically, do you measure the presence of an up-to-date Healthcare Power of Attorney (HC-POA) documentation, completed POLST form, or documentation of a Goals of Care Conversation? Do you consider all three components necessary for a complete advance care plan, or do you prioritize certain aspects over others?

    I'm not sure if this will help, Jennifer, but I've attached the measure specifications from the qpp.cms.gov website. For purposes of meeting this measure for MIPs, it only specifies 'Advance Care Planning discussed and documented', with a corresponding code.

    Each state has rules covering what is and Advance Directive. I see there are four types of advance directives in Illinois https://dph.illinois.gov/topics-services/health-care-regulation/nursing-homes/advance-directives.html#:~:text=Illinois%20law%20allows%20you%20to,%2DSustaining%20Treatment%20(POLST). 

    This is so important!
