Does anyone in their Ambulatory clinics provide actual care plans for each of their patients? We utilize our screening at each appointment as our plan of care. We screen the patients for Medication Rec, Falls, Allergies, If they feel safe at home, Domestic Violence, etc. and if any red flags pop up we alert a care manager or social worker to intervene. Here is the response from Joint Commission: it essentially leaves it up to the discretion of the facility, but interested if there is any EBP stating differently? Would love to compare any input you have. Thank you, MT
