Hello,

I have been asked to create Float Guides for RNs, LPNs, MAs, and PCTs who float in the Ambulatory setting in my region. This would be a one-pager with useful, role-relevant information specific to each clinic. I would like to create a template that each clinic can use to fill in according to their specific needs. Has anyone done this and would you be willing to share?

Attached is a tool that I created for use among our ambulatory team members. It is bit longer than the one pager you are looking to create and some items may not apply depending on how your sites and system are run. Feel free to use this as a starting point. We also ask that the site receiving the float provide introductions and quick site tour when then arrive (as noted in red).
