While our ambulatory footprint is not quite as large as yours, we have established a "train the trainer" program for some of our annual competencies.
For example, SME's (subject matter experts) are required in each practice who performs POC testing.
We validate the SME's competency annually, and they in turn sign off the staff in the local clinics. SME's can be MAs, LPNs, RNs- depending on the clinics' staffing models.
Depending on what annual competencies you require, I am sure some of them can be done virtually.
