Clinical Ladder for Ambulatory Nurses

  • 1.  Clinical Ladder for Ambulatory Nurses

    Our medical group Ambulatory RN's would like to start a Clinical Ladder. We are associated with a hospital system but are currently not part of the Clinical Ladder which is mostly developed for hospital nurses. Does anyone have an Ambulatory RN Clinical Ladder they have developed that they can share?  Thank you.

    I did a webinar for the Leadership SIG several years ago on how we leveraged our shared governance to develop a clinical ladder program for our Ambulatory nurses based on our professional practice model and the Magnet components.

    I’d be happy to share if you’re interested or I believe it may have been recorded and the presentation available on the AAACN website.

    We are all included in the same clinical ladder program.

    Yes we do. It was because our team is exempt and that was the only real reason. They said we can determine a ladder within our own department... honestly this is all new to me as this is the first time Ambulatory is included in Magnet and being pulled into other things. Other Ambulatory teams such as nurse triage is hourly so they are eligible for the ladder program.

    We are part of a hospital system as well but our Ambulatory Clinics are included in the clinical ladder called the Professional Portfolio Project.   I moved from the hospital to our Ambulatory Clinic and have completed it with both entities.  There are many process improvement opportunities in the ambulatory clinic and I found  choosing a project in the clinic is far easier and more fulfilling than it ever was in the hospital.  It is a shame they have excluded Ambulatory Care.  It really is a team effort to keep our communities healthy!

    Our hospital system also includes both the hospital and other outpatient/ambulatory clinics and departments in one clinical ladder program. It focuses on things that are done in all areas of the health system - topics such improvement projects, precepting, team building/recognition, involvement in shared governance and community involvement. We also don't provide additional pay for things like BSN or certification, so the program rolls these in as well by allowing nurses with these qualifications to move higher on the ladder.     We are part of a Magnet hospital system and our nurses in ambulatory advance on the same ladder that the inpatient nurses do. We had to make some recommended changes to the ladder to ensure equity with requirements and has worked well.
