Hello!
We are looking to implement a centralized group of remote community health workers that will be an extension of the care team. We plan to start with a focus on patients who have been identified as needing additional assistance through SDOH screening in primary care.
My question to you all is: How many CHWs do we need? Is there a ratio of CHW:patients or CHW: providers?
I have searched and i am not finding any literature on staffing models or the number of CHWs that an organization might need. Any insight is appreciated!
