Good morning,

Does your organization have a controlled substance agreement that patients must sign when being prescribed medications that fall within this class? Additionally, are there monitoring parameters your organization has in place for how often these patients must be seen, when they need to have a UDS, etc? Any information shared would be greatly appreciated!
