    Hello,

    Our Ambulatory falls committee are wanting to create a falls assessment tool that is outpatient-specific.  Does anyone use Morse or JHFRAT but modify it for the outpatient world....?

    I would love to know more :)

    We use the STEADI algorithm from the CDC, which was more geared towards outpatient than Morse.  There is quite a bit of information on the CDC website.

    We use the STEADI for patients greater than 65, and for patients less than 65 we use this 2 question screening to identify patients at risk for fall in the clinic.

    Good Morning, 

    Our organization use the MAHC-10.  The MAHC-10 is a validated fall risk assessment screening tool for primary care.  I have attached the Home Based Primary Care Fall Prevention and management Toolkit from the VA for reference.  I hope you find it helpful.
