AAACN is proud to be the recognized leader in advancing the practice, education, and research of Care Coordination and Transition Management (CCTM) for Registered Nurses in ambulatory care settings.
To continue this important work, the Publications and Online Education Review and Recommendations (POERRS) Task Force invites you to take part in a brief survey that will help shape the next generation of AAACN’s CCTM resources.
Your participation is critical.
We’re seeking input directly from nurse leaders overseeing education programs and nurses serving in care coordination roles—those who best understand what’s needed to support effective, patient-centered care across the continuum.
🕐 The survey takes about 5 minutes to complete and will remain open through Friday, November 21.
📢 Please also share it with colleagues who are engaged in care coordination or nurse education—your collective insight will have a direct impact on how AAACN moves CCTM forward.
