  • 1.  Medication Refill Protocol and Approach

    Hello -

    Our health system is investigating the integration of a medication refill protocol into our EHR (Epic User), and we are considering whether the refills will be handled within the clinic or centralized to some degree.  Two questions that I would love feedback from any user here:

    1. Do you currently use a medication refill protocol integrated into your EHR and if so, would you provide any feedback to a group exploring this option?
    2. Do you currently refill medications centrally or within individual clinics?  I would also appreciate any feedback you have about your current model when considering these two options.

    Hi Justin,
    We have a centralized process for 8 Clinics, soon to be adding 16 more.  We do not use a nurse protocol.  In general here is our process
    1.  Renewal request comes in via fax or electronically from pharmacy
    2. The centralized non-licensed team reviews the request.  If it is consistent with the current med list, they pend the renewal to the provider
    3. If it is not consistent or includes a controlled substance, the request goes to a centralized LPN team to track down the discrepancy or follow our standard process for renewal of controlled substances.  The LPNs pend the renewal to the provider after they determine what it should be.
    As we add the additional clinics our centralized team will be tasked with quality improvement efforts to determine the sources of the discrepancies and then work to reduce them, thus reducing the workload of this team.

    In our 8 clinics this has been working well for the past 5 years.
