My organization's quality committee has recognized that we have opportunities to do a better job ensuring medication review and reconciliation is completed in a meaningful way. I serve on the committee tasked to make a proposal for a better process and would appreciate hearing your organization's success stories.
- What is your organization's process to ensure that medication review and reconciliation are done?
- Do you have different processes for primary care and specialty care?
- Do your providers reconcile medications they have not prescribed? For example, at my organization we have primary care providers who are not comfortable removing a medication that was prescribed by a specialist on the patient's word and specialist that are reluctant to remove medications that do not pertain their expertise.
- What were the pain points recognized by clinical staff and providers?
- If you have instituted a change, what were your lessons learned?
