Shannon; There are lots of surveys out there! However, an absolute favorite is 'The Worlds Simplest and yet Complete Customer Satisfaction Survey' in the book 'Trying Hard is not Good Enough' page 77:
1. DId we treat you well? (numerical rating)
2. Did we help you with your problems? (numerical rating)
3. Why did you rate us this way? (open narrative)
4. How can we do better? (open narrative)
I have not yet found a question on longer surveys that would not be addressed with one of these four questions.
