Hi everyone! I'm an Ambulatory Policy Coordinator working on a policy about patient care transitions between providers within the same clinic. So far, I haven't found any institutional policies on this, most examples are about transitions from the ED to inpatient or similar. Does anyone have policies or SOPs they could share to help me build a solid foundation? Thanks a lot!
