    We are developing standing orders for RNs to refill certain medications. Are there any pediatric primary care offices in PA, DE, or NJ that have this workflow? Would you be willing to share your process, standing orders? Thank you!

    Unfortunately, I don't have an answer for you, but recently I saw a software solution that builds some logic in for approving refills.  Unfortunately, I can't remember the name of the company.  It allows for some customization and takes a lot of the work of developing the standing orders away from you.

    Our organization uses HealthCatalyst. It has been a game changer as it has hard stops on refills based on time to last visit, lab results, and other key criteria. It also clearly defines staff roles and responsibilities. It is integrated directly into our Epic build so the team can utilize it without changing screens/double documenting.

    Dear Lisa and all,

    Thank you for bringing this to the group and we are in the process as part of our in basket improvement project to move towards a centralized refill process using nursing with medication guidance that will enlist the Epic customization programs. Having a standing order to guide our nurses to help refill certain medications with guidance (labs, appts, etc) including classifications (cardiac, antihistamines, urology, psych, peds), and number of refills and certain meds to directly set up and pend to the provider are identified. I am attaching our draft medication guidance and we have come up with a specific workflow.
    My ask is who is using a centralized approach in the FQHC setting? If so what is the staffing and who are doing the refills? Or are the refills handled in the individualized clinic settings? I am trying to pilot this project in a region (two clinic settings but am getting pushback from the nurses) yet they are already doing refills to some level).

    There was an excellent presentation at AAACN conference titled "Improving efficiency and safety in Ambulatory care clinics by implementing electron medication refill protocols"  The speaker was Jordan Serwe, she represented Advocate Aurora.  They have a well established program.  At Children's WI, we use Epic Refill Protocols, and have been able to build in parameters for identified medications per department.

    Hi Kimberly,

    We use Epic and have a Standing Orders policy that allows staff to approve refills for medications, provided that it passes the checklist.  I have attached a copy of the approved medications and a copy of the training video is below:
