    When do you close out referrals that have not been addressed (whether due to patient or refer-to location, etc)? 
    For those in a health system, when do you close EXTERNAL referrals? 
    Any other best practices on referral management?

    Hello!

    I work for an FQHC and we have a standing for addressing outgoing routine referrals.  Referrals expire one year after the referral (order) date.  Within two weeks of the expiration date, we make three attempts to contact the patient (via telephone call, email, SMS text) and if we do not get e response, we mail a letter to the patient.  If still no response after five business days, then the referral is closed.

    Hey There Sherrie!

    We close after 91 days or three attempts,2 calls and 1 letter,  (with a 14 day waiting period) before closing the referral for "failed mandated scheduling effort".
