    Good afternoon!  Wondering what kinds of RN roles are doing remote work? For those roles, what benefits have been seen and has there been any pitfalls for those roles that provide direct patient care. Thank you!

    Hi Larissa, 

    In our pediatric primary care clinic our nurses function in a hybrid role. 50% of their time is spent remote completing triage calls, prior authorizations, answering MyChart messages and processing refills. The other 50% of their time is spent in clinic performing nurse-related visits, in person education with parents and families, and supporting procedures that cannot be performed by our medical assistants. We have seen a huge decrease in nurse turnover, increased staff satisfaction, and greater role clarity with our nurses. Happy to discuss this workflow redesign with you further!

    We are an FQHC and doing some remote RN work, primarily intake work for new patients and follow ups for our transitions of care (ED post hospital) patients. We have done some remote work for the phone call/refill management, but it's not a standard process. I would be interested in seeing workflow to organize the work that have been successful.

    Audrey,

    I currently float between a pediatric clinic and a family med clinic. Although 95% of my time is spent triaging calls, answer portals, and other computer work, we are only allowed to work remotely 20% of our shifts. I am looking for ways to increased what we do in the clinic. Could you provide more information on the type of nurse-related visits, education, and other procedures your staff do in clinic? We do Newborn education visits and diabetic education/semaglutide training visits, but I'd love to have more ideas to give my docs to get more in-clinic activities.

    Good morning! We are a smaller organization, with 4 Primary Care Clinics and 2 Student Based Health Centers. Currently, we have two Triage RNs and an LPN, who does nurse intake visits, working in hybrid roles.  For these roles, that means that they are at the admin building 1 day a week and work from home their other days.  Their work is 100% telephone based so working from home works for us.  I like to have them come in to the office one a week for relationship building and oversite, plus I feel like it is hard to get someone to come back in, if needed, when they are 100% remote.  

    Overall, I haven't seen many pitfalls.  We do meet frequently as an entire team, so that is really helps build relationships.  There are the occasional technology issues that can be harder to navigate if they aren't on site.  I also think that managing staff remotely can be more difficult. 

    I believe that there have been many benefits.  First off, we have not had a hard time hiring, unlike my peers in the area.  There are nurses out there that want these roles. Second, I have found that nurses who have roles on the phone and are housed in a clinic, often get pulled into other tasks.  Then it leaves their main work at risk.  Lastly, if you haven't had a chance to read the article in the AAACN View Point from March/April 2023 entitled "Multitasking: Is it Work the Risk?", I think it is worth the read.  I like that having these nurses work from home is allowing us to decrease some of the multitasking risks.

    Hi Larissa,

    We are a fairly small group that completes after hour telephone triage calls for pediatricians associated with our medium sized medical center. There are 19 RNs in our department. All of our staff nurses work 100% remote. The nurse manager will work 1-2 days a week on site at the medical center and 3-4 days remotely. We are not unionized. We have been a remote department since 2011. We no longer have a call center at all in the medical center. Overall, our situation works very well and the nurses love being able to work remotely. Our retention is excellent and when a position is available, there are a large number of applicants. I would say we do not miss a call center except when we are training new nurses to our department. It is my opinion that new telephone triage nurses would benefit greatly benefit from having coworkers within arm's length to refer to, ask for guidance, or reassurance during difficult or puzzling call scenarios. We do stay connected during work hours with Instant Messaging and 1:1 phone calls, but we are always searching for the best practices to support new nurses when they join our group.

    We are a Telephone Triage department with 40 RN's, all of whom work remotely. We also have low staff turnover; however, it can be difficult to engage staff who are strictly remote, and also hard to monitor at times. Please let me know if you have additional questions.

    Some of the roles approved for RN Remote work include: Clinical Call Center (CCC) Triage RN's, Clinical Resource Hub (CRH) telehealth RN's, hybrid Primary Care RN's within the Veteran's Health Administration. Some of the benefits we have observed include 1) Employee Satisfaction 2) Space saving in crowded clinics 3) Increased Virtual Visits 4) Opportunities to provide virtual care to patients despite bad weather. We piloted different models allowing each member of the Primary Care team to work virtually 2 days a pay period and this has been positive for employees and patients. There is oversight that needs to be in place along with clear expectations for accountability. A remote work agreement needs to be in place with regular check ins for this to be successful as well!
