    We have a chart for billing Stand Alone Nurse visits.  Our practice is in Massachusetts and this may not be applicable to all but may provide some help and guidelines.

    Moving forward, be sure to differentiate between the RN (nurse-led) visit and the APN (nurse-led) visit. 

    1. Currently, the "incident to" billing is the only code (99211) that can be used for the RN only visit. It is a visit that must be previously documented in the EHR as medically necessary continuance of care by the eligible provider. This requires greater "voluntary" collaboration between medicine and  RNs. 

    2. The APN is an "eligible provider" according to CMS and can bill 99212 through 99215 for any established  encounter and 99202 through 99205 for new patient encounters. Many state statutes require mandatory collaboration  (written protocols) between medicine and APNs.
