I've been hearing from multiple institutions that the front desk staff generates the AVS. I imagine for some orders and referrals the front desk staff is capable of reviewing with the patient. But for some medications and orders, I imagine this is not ideal.

Providers are responsible to sign orders, before checkout regardless of who is placing those orders in my state.
The provider can sign for those activities without completing note using our EMR (EPIC). Orders, referrals and medication review can then be added to AVS. Once signed it takes 2 additional "clicks" for the provider to place on AVS.
This said, I would expect initial pushback from the providers, change is never easy.

This week, I noticed that many of my organization's standardization efforts have focused on intake/registration/rooming, all front-end processes.
What seems to be lacking is standardization in the "discharge" process, on end of visit actitivites (reviewing new medications, referrals, pending authorizations, follow-up visits, etc.)
There are challenges there, such as not having nurses at every site, providers not utilizing after-visit summaries because they haven't finished notes, etc.
Just wondering if anyone has a robust "discharge" process they would be willing to share. Might be a good start for a gap analysis or a future state map.
