    Please see attached. The job description is templated so no changes can be made to the MJRs and essential functions,  but the job summary is left blank for clinics to individualize. We require the NTL to spend ~60% of their time in the clinic caring for patients and the other ~40% administrative/supervisory duties. They must have a BSN to be eligible for this role and must advance on our clinical ladder within 2 years of accepting the position. Let me know if any questions.

    Job Title: Lead Clinic Registered Nurse

    SUMMARY

    The Lead Clinic Registered Nurse delivers nursing care to Neighborhood Health Center (NHC) patients in a Primary Care Clinic. This role is works collaboratively with all members of the healthcare team and requires the ability to apply the nursing process (assessment, diagnosis, planning, implementation, and evaluation) to all care provided. Registered Nurses are expected to understand and follow the Oregon Nurse Practice Act as administered by the Oregon State Board of Nursing and practice according to their scope of practice. Under the support and management of the Nursing Manager and Clinic Manager, the Lead Clinic Registered Nurse supports and provides leadership to the nurses. They are responsible for the daily operations of the nursing team, including planning, coordinating, and evaluating nursing activities.

    Essential Job Duties

    Meets weekly, and as needed, with other Lead Clinic Registered Nurse.

    Escalates concerns appropriately to Nurse Manager and Clinic Manager.

    Attends daily clinic huddles.

    Facilitates daily nurse huddle with assigned nursing team.

    Develops and communicates weekly nurse schedule and daily edits as needed.

    Monitors nurse Teams chats and answers appropriately.

    Support colleagues with resources, guidance, and hands-on collaboration as necessary.

    Manage shift safety, adherence to policies, and compliance with regulations.

    Mentoring and training new staff and providing support and guidance to all staff members.

    Coordinating daily administrative duties, including schedules, nursing assignments, and patient care.

    Creates and delivers training for new and existing nurses and medical assistants who need further training or support.

    Develops and/or delivers clinical updates to nurses and medical assistants as appropriate.

    Performs audits of workflows according to NHC standard work.

    Collaborates with Nursing Manager and Clinic Manager on the day-to-day operations and performance of nurses.

    Collaborates with Nursing Manager, Clinic Manager, and other clinic leads to create standard work, checklists, and job aides for nursing.

    Delegates tasks and patient assignments to nurses.

    Monitors Epic in-baskets and directs coverage for in-basket work.

    Monitors and reports on team defined Key Performance Indicators to Nursing Manager and Clinic Manager.

    Attends organizational meetings as delegated by the Nursing Manager.

    Effectively manages assigned projects in collaboration with key stakeholders and relevant staff.

    Performs comprehensive health assessment of patients by completing a physical and psychosocial history. Identifies actual and potential health needs based on medical, mental/emotional, financial, and social factors.

    Triages minor to emergent situations.

    Respond to medical emergencies.

    Performs nurse visits and administers medications and immunizations as ordered by an authorizing NHC provider or via NHC approved standing order.

    Assists patient to maintain maximum quality of life by coordinating services of community agencies and encouraging preventive practices

    May assist with phlebotomy, CLIA waived point of care testing, in clinic EKGs and other primary care procedures as necessary in times of staffing constraints.

    Performs RN led patient visits for health maintenance, patient education, and treatment of acute issues based off protocols and provider orders.

    Consults and coordinates with health center team members to develop and implement comprehensive patient care plan based on assessment.

    Assists patient to establish short and long-term goals to obtain optimum level of health through advocacy, counseling, teaching, problem solving, and selection and utilization of resources.

    Instructs individuals, families, and other groups on topics such as health education, disease prevention, and develops health improvement programs.

    Monitors, Plans, implements, and evaluates case coordination and management for patients with chronic illnesses.

    Participates in team huddles to review the schedule at the start of the workday.

    Participation in local vaccine management.

    Accurately, concisely, and succinctly documents in the medical record all patient contact.

    Manage an electronic in-basket of incoming and outgoing patient calls, MyChart messages, refill requests, and other messages that directly impact patient care plans.

    Implements required nursing procedures consistent with NHC procedures/protocol.

    Participates in process and quality improvement workflows.

    Remain calm and effective in high pressure and emergency situations.

    Assists in new employee training, as needed.

    Maintain punctual and predictable attendance.

    Performs other duties as assigned.

    QUALIFICATIONS

    Education and/or Experience

    This position requires 1-2 years of previous Lead experience in primary care or 1-2 years of experience at NHC as a Clinic Registered Nurse.

    This position requires a bachelor's degree of Science in Nursing.

    This position requires completion from an accredited school of nursing.

    License and/or Certification Requirements

    This position requires a current unrestricted Oregon RN license.

    Active CPR certification required.

    National Provider Identifier (NPI) required.

    Knowledge, Skills, Abilities & Behaviors

    Strong communication skills.

    Has strong foundational clinic knowledge and skills on which to base judgments and make decisions.

    Maintains composure under stressful situations.

    WORKING CONDITIONS

    Work Environment

    The noise level in the work environment is usually moderate to loud based on patient volume.

    As a result of high patient volume, this position experiences stressors that come along with a fast-paced environment.

    Must be able to travel during the workday and/or cover at other locations as needed.

    While performing the duties of this job, the employee is occasionally exposed to toxic chemicals or blood borne pathogens and regularly handling contaminated sharp instruments.

    Good morning,

     

    Very comprehensive list of responsibilities. I'd be interested to see what the Nursing Manager or Assistant Manager are responsible for. At my organization, many of the responsibilities you listed are done by the manager and assistant manager. A Team Lead Role may be an interesting concept to implement.

    Hi Rebecca, 

    Here is the list of what our Nurse Manager does.  As a medium sized organization we don't have a separate educator or infection control department, so you will see some of those items added in.

    ·        Ensures delivery of comprehensive patient care in accordance with nursing standard work and established nursing policies and procedures.

    • Provides support to the clinical team for ongoing training and skills validation in partnership with other NHC trainers and leaders.

    ·        Performs interviews, hiring/firing, and corrective action activities for designated nursing positions in partnership with the Director of Nursing and Human Resources.

    ·        Conducts regular and timely performance reviews that encourage development and / or develops performance plans as needed.

    ·        Creates an environment to promote ongoing learning and personal growth amongst the Nursing Team; mentors nursing staff to help them meet their         professional goals.

    ·        Conducts routine one on one meetings at each Nurses' worksite.

    ·        Conducts standard work audits to ensure Nursing Team member compliance to standard work.

    ·        Collaborates with the Director of Nursing and other leaders to develop, implement, and update nursing procedures and standard work.

    ·        Conducts Nursing Team meetings for effective team and organizational communication.

    ·        Recognizes and responds to operational opportunities and the need for additional staffing, training, etc.

    Good morning,

    Our lead reports to our Nurse Manager. I have also worked in places where the lead reports to the practice manager.  Since the practice manager wasn't required to be an RN, this feels like a cleaner reporting line to me.  Where at all possible, I think there is great benefit if RNs report to an RN.  Of course there are challenges of the Nurse Manager not being on site daily but we have navigated that with frequent checks with the RNs and the practice managers.

    My nurse team leads in almost all cases report to the nurse manager. I have a few smaller clinics where they report directly to the practice manager (non-nurse). In those cases they have a dotted line to me as the CNO.

    Does your organizing utilize Clinical Team Leads who are not registered nurses? For example a certified medical assistant with an over arching nurse manager across many clinics?

    All our clinical supervisors are RNs.  The clinics skill mix are RNs, LPNs and MAs.  Some divisions have RN managers.  The clinical supervisors report up to them.

    We do have Lead MAs at every site as well.  They report to the clinic manager but we also have an MA Program Manager that reports to me.
