    Hello,

    Does any institution have a policy on ambulatory walk-in triage?  Ours is up for renewal and when I reviewed it, it shows that we should be using ESI, which is more for ER triage RNs.  I couldn't find anything in the literature specific to triaging ambulatory walk in patients.  Can anyone share with me your current process for a walk in patient who requires triaging?  

    Do you have an RN on site?  What is the workflow?  Do you have a policy for this?

    Any feedback and guidance is appreciated!

    Hi Cindy. 

    I work in a family practice and we do get the occasional walk-in, unscheduled patient who needs to be triaged.

    I'm not aware of any written policy re: walk-in triages but I do think that's a good idea to have one. 

    When this situation comes up an announcement is made by the front desk check-in staff person " Triage Front Desk ". An RN responds.

    If the patient appears stable enough they are brought into the first exam room and the nurse starts a " Telephone " encounter. In EPIC there is the option to change it to an " In person " encounter.

    The RN quickly asks the patient about complaints and symptoms. Vital signs are taken if the situation warrants it. EVERYTHING is documented, just as you would do for a telephone triage encounter.

    The outcome depends on the seriousness of the patient's symptoms. The possibilities include an office visit right away with a mid-level provider, a later same day appointment for a less severe situation, or calling for an ambulance if the patient needs a higher level of care.

    If the patient has very serious symptoms the RN will consult with a physician of course.

    We have a sign on the door stating that patients need to have an appointment but that doesn't stop patients from walking in without an appointment anyway.

    I have even had someone walk in who wasn't even a patient in our practice. He was jogging by and was having chest pain. Of course we called 911 but luckily he also agreed to be enrolled as a patient while he was being assessed. This helped me as an RN and I was able to document the whole situation in EPIC. The patient ended up needing coronary stents and he came back for a follow up. 

    I would imagine that if the person is not an established patient and they don't agree to be enrolled as a new patient an incident report would need to be submitted as documentation of the situation. 

    I would like to hear what other nurses have to say about walk-in triages.

    Hi Karen,

    Thank you for sharing this feedback.  I'm glad to hear you were able to react quickly and help that gentleman.  Some of our clinics do not have RNs on site (some work remotely answering calls in another building) and some clinics have non-nursing managers.  If it's a specialty clinic, the providers aren't there every day.  There are times when patients "walk in" the clinics for non-urgent requests because they don't want to wait for a nurse to call them back or because they don't to wait in the ED.

    This can cause disruption to the clinic workflow and I do hope that if we have a more defined guidelines, it will make it more efficient for the clinic staff.

    We have a policy for Triage Outside of an ER with Standard Work for triaging a patient who walks-in with "red flag" symptoms. I've attached copies of both for you to review.

    Hi Cindy:

    I am a Triage RN in a busy Outpatient Cardiology Clinic at the University of Vermont Medical Center.  This is a timely topic as we have gotten a few walk-ins lately who are in distress.  It can be difficult when they are not a patient at the clinic, and don't want to go to the ED.  People outside of our arena don't understand the complexity of the clinic and that we aren't a mini ED.  Reviewing the posted policies is quite interesting and gives food for thought.  At this time we are in the midst of formalizing a clinic policy.  I look forward to reading more.

    Hi Cindy,

    My organization is working on a workflow for Ambulatory Care Walk-Ins. They do happen quite frequently for us in our health system.

    The strategy most of our clinics have is that an RN has to triage a patient who is either wanting to book an appointment due to symptoms, or just if the patient is asking to talk to the nurse. We try to apply the telephone triage protocols from Barton and Schmidt to help with nursing decision making, but sometimes the nurse will need to discuss it with the provider to see if they need to squeeze the patient in or get them into an acute visit. I too have triaged some serious walk-ins and have had to call rescue. If a clinic doesn't have an RN, and had an MA only, they might have to involve a provider with decision making. This is something we don't have a policy about yet but is a hot topic for our organization. We need to develop a policy, so we have standardization, and to protect the staff due to liability.

    Hello!

    Our organization took telephone triage out of the sites a few years ago but many of our clinics are very neighborhood-based so they continue to receive walk-ins frequently though we are not "walk-in" clinics. A  practice guideline was recently enacted that helps provide guidance to our front desk staff (PMR's) as well as outlining expectations of the nursing team(s). I've attached that here.
