Dear CCTM SIG Members,
It is my sincere pleasure to serve as the Chair of the Care Coordination and Transition Management Special Interest Group (CCTM SIG). I look forward to engaging with you as we chart the course for the future of CCTM SIG together.
As a member, you recognize the critical role CCTM nurses play is enhancing access, safety, and quality of care. So many of us have witnessed improved clinical outcomes, improving patient/family/provider satisfaction when care is delivered by multidisciplinary teams working across all settings. In this challenging time for healthcare, it's more important than ever that we share best practices and standardize care delivery to achieve high quality, cost-effective care built on strong cross settings partnerships.

· Networking Call – we have monthly networking calls on the third Wednesday of the month from 12:00 -12:45 PM EST, starting August 20, 2025. Please join us on a monthly basis and let me know topics you would like to discuss!
· CCTM Advisory Council – We are creating a CCTM Advisory Council that will meet bimonthly. The goal will be to create a team representing a cross section of CCTM nurses from across the country, representing the diversity of our membership in location, experience, interest, and enthusiasm to lead care transformation and payment reform. If you are interested in serving, please submit a letter of interest and your CV to Billielynn33@gmail.com by August 20th.
· A member survey will also be posted soon on the CCTM SIG Connected Community to gather feedback on your priorities and interests, including topics for education and webinars. Please feel free to submit additional questions or suggestions for the survey via Connected Community.
· A plan is underway to update the CCTM Scope and Standards of Practice with support from the AAACN Publications Committee. More information will follow regarding opportunities to participate in this project.
· At the 2025 AAACN Annual Meeting, I met many attendees and look forward to interacting with others at future events. I feel blessed to have this opportunity as I believe that the work we all do is critical to the future of healthcare.
