    Hello!
    I am looking to connect on the following: 
    We are working on evaluating the current structure/function of wound care across our system in all entities (hospitals, clinics, home health) and exploring options of the best way to deliver care to these patients across the continuum.

    - Are there any large health systems (hospital/office-clinic/home health) with a wound care/consult service addressing patients across the care continuum? 
    - How are patients with wound or pressure related injuries addressed/cared for (WOCN utilization? WTA? Role/scope/practice expectations for nursing)
    - How are nurses educated to care for patients with wounds (specific programming, continuing education/comps?)
    - What outcomes are you tracking for wound patients/how?
    - Do you have specialty wound clinics, or patients cared for in primary care with wounds, or both?
