  • 1.  99211- Billing Nurse Visits

    Hi all,

    Curious if your organization uses 99211 (CPT code) for patient visits. If so, what are the procedures performed during this visit?  Currently, my organization does not bill for return BP checks. Wondering if we are missing out on this opportunity.

    ANY nurse visit is billed with 99211. 

    Our organization is moving away from this and requiring all of the nursing visits to be booked under a physician, and a physician must see the patient and will be booked as 99212. Sadly I don't control the Navy and DHA. 

    We charge for every encounter. Suture removal, protocols (diaper rash, N/V, UTI, Cold Sxs, etc), blood pressure checks, Depo shots/med admin, ear lavage, etc.

    We bill for RN visit if it includes patient education (well documented), B/P check, Medicare Annual Wellness Visits, A1C and Diabetes Ed. Our facility is really engaging the RN to support the patient care.

    Hello,

    We have a structure for this at our organization in Cleveland. We do use nurses for billable visits for task work such as suture removals, BP checks, Diabetes education, vaccinations and medication administrations. When tasks are coupled with education and/ or care coordination, we do bill for the visit with a 99211. It provides a mode of access for patient care needs and helps to save provider visits for more acute or in-depth types of visits.

    If your patient has HTN and is scheduled for a follow up blood pressure check that was documented as necessary by the previous provider visit, you are leaving money on the table and using your RN as part of the room rate instead of as a skilled reimbursable provider . 99211 is a type of incident -to billing. There must be a provider visit that indicates the need (medical necessity)  for the follow up blood pressure checks.

    This is a good start for demonstrating nursing value but I dream of the day we didn't need to bill these visits as incident-to.

    We met with our billing team and there are elements of documentation required to bill for nursing visits.  This is required to pass audit.  This is a process we are currently working on to identify billable services and standardize across service lines.

    When I was employed by a hospital group in Primary Care, we did bill out 99211 visits whenever a patient was seen by a RN and education was completed or an assessment was required,

    Now that I work for a FQHC organization, we do not bill out 99211 visits, as I have been told we cannot (although I am still trying to understand this)
