    I have only worked at a free and charitable clinic. We do not make appointments. Everyone seen on a first come, first serve basis.

    Hello,

    For our primary care clinics, we have an RN triage the walk-in patient using protocols established in partnership with the physicians. If the patient is non-emergent/urgent, then a same-day appointment (any available physician) or next day appointment is scheduled. I hope this is helpful

    The Internal Medicine & Pediatric clinic I work for also follows a similar process for any walk-in patients. We try to assign one RN each day to see these patients and have them doing work off the phones so the rest of us can focus on in-coming calls. Our front desk staff will initially ask the reason for the patient walking in, but if it's something that a message can be taken for (like non-urgent refill), they will send a message to the care team for it or if it's a non-patient care need (such as a form), then they will get a team coordinator to assist them.

    I work in OBGYN but we get a fair amount of walk ins since we are on a college campus and service students and employees. The front desk staff will contact the triage nurse. The triage nurse will triage the patient and decide if the complaint warrants a same day visit fit in or can be scheduled within the week. Some patients request a pregnancy test or emergency contraception and the triage nurse can take care of that.

    I work at an FQHC in Vermont. Our system contracts with a company that provides NPs and PAs to do walk-in services. We provide clinical support and rooms for them to see patients. Generally, it is FCFS unless someone comes in with obvious distress or complaints of chest pain or SOB. If the front desk staff thinks it might be an urgent issue an RN is asked to triage the patient. If needed the patient is roomed immediately and the walk-in provider is grabbed to see the patient. We do have wait times that exceed 2 hours pretty frequently, but the next closest clinic is 30 minutes away and we are next to a ski resort, so we get very busy in the winter.

    For our primary care clinics if a pt walks in and has an acute need, a nurse will triage them and either have them seen in clinic or sent to our acute care clinic.  During COVID we opened an acute care clinic (not urgent care) that now sees patients with a needs such as sore throat, UTI, HTN and cannot get an appt that day with their PCP.
