  • 1.  Ambulatory Care Plans

    Does anyone in their Ambulatory clinics provide actual care plans for each of their patients? We utilize our screening at each appointment as our plan of care. We screen the patients for Medication Rec, Falls, Allergies, If they feel safe at home, Domestic Violence, etc. and if any red flags pop up we alert a care manager or social worker to intervene. Here is the response from Joint Commission:  it essentially leaves it up to the discretion of the facility, but interested if there is any EBP stating differently? Would love to compare any input you have.  Thank you, MT

    Were you able to get any care plans, we are too looking at care plan in regards to chronic care conditions.  If you do, any that you can share.  Much appreciated.

    We do not use care plans, per se. We use "clinical reminders", which helps individualize the plan of care. This is based on their age, gender, and health history. We service a special population, so we address issues that are Veteran-centered. As an example, we know our female patients should have an annual PAP and mammogram. The reminder is satisfied once completed, then it is reactivated so their PCP will order for the next annual exam . Also, we use a patient-centered approach to care that focuses on chronic disease management; health promotion disease prevention (HPDP). I would think all of this is EBP; however, I do not have the specifics. Perhaps this is something we can research and publish.  

    I can reach out to get a full list of our reminders from our informatics RN, and you might be able to use it to craft something specific for your organization. I can also contact our HPDP Program Coordinator for more information.

    We use a Pop Health tool that interacts with our EMR - we use Elation- we recently introduced Clinical reminders once the provider or nurse note is open to highlight missing health screenings the member qualifies for .

    the Pop Health tool is also organized to reflect data that is beneficial for outreach as it can provide a snapshot view of last APE and breaks down all the chronic conditions as well as the SDOH info last managed

    In our Ministry @OSF HEALTHCARE,  we utilize the Elsevier Care Plans. We start a program in our Ambulatory Nurse Clinic. We assess for healthy lifestyle, health conditions, safety, and abuse. Based on the patient responses Care Plan options populate. Once the Care plan is initiated the patient will receive notifications regarding tasks to complete via their MyChart. The only draw back that I have found is that patients without MyChart or whom don't utilize their MyChart do not get the benefit of Elsevier Care Planning.
