Our organization practices in Epic and we are looking at trying to make patient mobility and transfer needs, as well as fall risk more apparent in the chart. I am curious if there are others who have implemented a fall risk assessment or mobility/transfer needs process into their care workflows, where this gets documented and how it is visualized within your Epic system where everyone can see the patient needs assistance with mobility & transfers.
