  • 1.  Botox Preparation

    Good Morning
    Currently we administer Botox in our neurology, urology, and reconstructive surgery practices. Some of those offices use RN's to mix and administer the drug. One of our practices only has Medical Assistants as part of the clinical teams, no nurses.  Currently, we have restricted our MA's from preparing this med. I am wondering if any of you have similar rules or if you allow all levels (CMA's, LPN's and RN's) to prepare Botox?

    Hi Shannon,

    In a previous role I held (in MA), we administered Botox for migraines. They did not allow anyone other than the doctor, NP, or the RN to prepare Botox. I'm unsure if this was a state law or a hospital policy. If the RN was unavailable or couldn't help in Botox clinic that day for whatever reason, the doctor prepped it. The doctors of course didn't like this policy, but it was determined that it was best practice.

    We allow RNs/LPNs/CMAs to reconstitute Botox.

    MA scope of practice is a sensitive issue and is state by state-similar to other scopes of practice. Really depends on interpretation of COMAR regulations. What I have seen is single-dose medications not requiring measurement or reconstitution are able to be administered by MAs. Any medication requiring measurement, assessment before or after, or on a hazardous medication list would be administered by licensed healthcare workers.

    Hello Shannon,
    If you read the scope of practice for the MA as indicated by your individual State's Booard of Medicine you will find that MAs practice is limited to the administration of unit dose medications, and never the adminsitration of any anesthetic agent.

    If a dosage has to be adjusted or calculated for administration that is considered outside of the scope of practice for a medical assistant.  I do not allow MAs to draw up any medications in my practice. However, some of individual physician providers will. It is a provider's individual their right to take that risk amd responsibility, but It may not be the policy or type of risk your institution or practice wants to take.
